I hereby acknowledge and state the following:
I have chosen to volunteer to feed hungry people and make a difference in the neighborhoods of Uptown, Ravenswood and Albany Park. I understand that I am spending the day as a volunteer for Ravenswood Community Services (RCS) and/or All Saints Episcopal Church (ASEC) and that there are risks and dangers inherent in my participating in this event or serving as a volunteer.
I attest that I am physically capable of participating in this event or serving as a volunteer. I understand that my execution of this Waiver and Release is a prerequisite for my participation in this event or my serving as a volunteer for RCS and/or ASEC.
I hereby assume any risk of loss, property damage, or personal injury, including death, that may be sustained by me, or any loss or damage to property owned by me, as a result of my participation in this event or my serving as a volunteer, whether cause by the negligence of the Released Parties (as hereinafter defined) or otherwise.
I release, waive, discharge and covenant not to sue RCS and ASEC and their officers, directors, employees, agents, affiliates, as well as any event sponsors or other participating organizations and their respective officers, directors, employees, and agents (collectively, the “Released Parties”) for any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to death, personal injury, or any other loss, injury, or damage of any kind whatsoever sustained to my person or property during my participation in this event or while serving as a volunteer, whether caused by the negligence of the Released Parties or otherwise.
I agree to indemnify, defend and hold harmless the Released Parties form any loss, liability, damage or costs, including attorney’s fees and court costs, that maybe incurred due to my participation in this event or my serving as a volunteer, whether cause by the negligence of the Released Parties or otherwise.
I understand and agree that this waiver and release is binding on my heirs, assigns, and legal representatives. I grant full permission for organizers to use photographs and/or video of me and/or quotations from me and/or my name, without compensation, in accounts and promotions of this event and/or RCS and ASEC in perpetuity and in all forms of media now and forever known.
I understand the need for confidentiality and will not discuss, photograph, or otherwise disclose identifying information about the neighbors seeking help from RCS/ASEC without prior permission from RCS, including any references to names, addresses, or other identifying information.
In signing this Waiver and Release, I acknowledge and represent that (1) I have read the foregoing Waiver and Release, understand it, and sign it voluntarily as my own free act and deed; (2) no oral representations, statements, or inducements, apart from the foregoing written agreement have been made, (3) I am at least eighteen (18) years of age and fully competent (or if less than 18 years of age, a parent, guardian, or chaperone has signed this agreement next to my signature); and (4) I intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
I may choose to participate in additional activities on multiple or later dates, and this Volunteer Agreement and Release will apply to any RCS activities in which I participate in the future.
All members of your group must sign-in here. Please add your Full Legal Name under “Volunteer Name” and Parent or Guardian name if the volunteer is under 18.
Ravenswood Community Services is committed to protecting the confidences and confidentiality of those we serve. In order for those we meet to feel safe and willing to form ongoing relationships, they must feel they can trust all associated with RCS. We expect all staff, volunteers, and guests to maintain the confidentiality of our neighbors. This means:
1) You may share the content of your encounters with RCS staff present at our Tuesday ngiht community kitchen and food pantry. You may want to share some of your experiences with friends, family, and/or co-workers. Please do this in a way such that the person about whom you are speaking cannot be identified.
2) NEVER share information about one neighbor with another neighbor.
3) When you are in other settings, do not acknowledge neighbors that you have met through your service at the community kitchen or food pantry, unless they acknowledge you first.
4) NEVER disclose the names of our neighbors to anyone until cleared by a staff member of RCS.
